Reentry Support and Other Services
Preventive, Primary & Behavioral Healthcare Coordination Coordination of parish-level integrated prevention, primary and behavioral health care services provided through clinics and public health units by community-based providers for CAHS clients. Screening, Brief Intervention, Referral and Treatment (SBIRT) Program: Co-Location of Addiction and Behavioral Services in Prenatal Care A multi-disciplinary evidence-based SBIRT approach to promote optimal behavioral health of pregnant women to reduce the harmful effects of substance use, depression and domestic violence and improve birth outcomes. Wellness Services Comprehensive wellness services, including health education, tobacco cessation groups, nutrition classes, and physical fitness activities for CAHS clients. Nurse Family Partnership Evidence-based community health program providing home visitation services to first-time other who are less than 28 weeks pregnant and their families through the child’s second birthday. Post-Incarceration Service Coordination Coordination of screening and discharge planning to assure timely access to services upon release/reentry into the community. Emergency and Disaster Preparedness Coordination and response services of people with behavioral health needs, development disabilities and the broader community to expedite attaining a safe environment and assist in recovery from emergencies/disasters. Behavioral Health Emergency Services Continuum A coordinated comprehensive community-based system to avoid repeated cycles of crises and to support stability. Peer Supported Coordinated Services Coaching and mentoring by certified peer support.
