Program
Retention In Care/Second Chance
The Retention In Care Initiative focuses on formerly incarcerated HIV positive individuals. The program is design to provide our target population with the tools to enhance their quality of life and reduce recidivism. The components of the program are employment readiness, employment search and job placement. Clients also will be offered GED Prep by a retired teacher with experience teaching in a Correctional setting. This program addresses the substance abuse and/or mental health issues that accompany individuals living with HIV/AIDS through individual and group counseling by a Licensed Clinical Social Worker (LCSW). Clients will have access to a Patient Health Navigator that will help clients identify and address those barriers that may prevent them from remaining in care.
